Medical matters and official complaints cannot be dealt with via this form. If you have a query regarding a medical matter please telephone reception to make an appointment to see the appropriate person.

We make every effort to give the best service possible to everyone who attends our practice.
However, we are aware that things can go wrong resulting in a patient feeling that they have a genuine cause for complaint. If this is so, we would wish for the matter to be settled as quickly, and as amicably, as possible.
You have the option to complain either to us directly here at Foxhill Medical Centre or NHS England.
If you wish to make your complaint direct to us, you can complete this online form, or put your complaint in writing to us, or phone us on 0114 2322055. If you would prefer to arrange a video call, please let us know and we can arrange this for you.
